  • Access is carried out through the Internet, on the website enabled by the Health Service with the Individual Health Card (TSI).
  • For clinical information to be accessible, the Health Service must have enabled the issuance of clinical documents.
  • Citizens must identify themselves through the electronic means offered by their Autonomous Community, such as electronic ID card, an electronic certificate issued by a trusted certification authority, or agreed-upon passwords through the Cl@ve platform.

  • Some possible reasons are detailed below:
    • The information is old and predates the computerization of clinical documents, therefore it is not recorded in the Electronic Health Record system of your Health Service.
    • The clinical document may be in the local information system of the healthcare center where the individual was treated. However, it is possible that it has not been sent to the Electronic Health Record system of your Health Service.
    • The Health Service that has that information is pending publication in HCDSNS.
    • The report belongs to one of the typologies that, at the moment, are not available in the Health Service. To see the reports offered by your autonomous community click here.

  •  After accessing the system, on the page that contains the index of available documents, you should locate the button, icon, or check box labelled "Hide" next to each document.
  • Upon clicking the button or icon, a screen will display information that includes a warning about healthcare professionals potentially needing to access hidden documents in certain circumstances (emergency access).
  •  The concealment is reversible, also acting through the home page.

  • On the main page, through the "Access Log" button or icon, you can obtain a list of the accesses to your HCDSNS documents.

  • First, you need to identify the access for which you want to file a complaint.
  •  Through the "Complaint" button or icon, activate the electronic "Access Complaint" form. You must complete the information in the complaint form, including your contact details.
  •  After submitting the form, the complaint page will provide you with an acknowledgement of receipt.
